Key Responsibilities
Clinical Care
- Chronic disease management support
- GP management plans & care coordination
- Health assessments
- Immunisations & preventative healthcare
- Wound care & clinical procedures
- Patient education & health promotion
- Monitoring patient outcomes
Team Collaboration
- Work closely with GPs & allied health providers
- Coordinate patient care plans
- Maintain accurate clinical records
- Support quality improvement initiatives
- Contribute to a positive team environment
Who We Are Looking For
Essential Requirements
- Current AHPRA registration as an RN or EN
- Chronic disease management & care planning experience
- Excellent organisational abilities
- Ability to work independently and within a multidisciplinary team
- Experience in a general practice environment
- Strong communication & interpersonal skills
- Professional & patient-focused attitude
- Commitment to continuous learning & development
